We are seeking qualified Nurse Practitioner that specializes in Behavioral Health to service our patients through the state of Georgia. As a Provider, you play a critical role in delivering exceptional patient care through telehealth. You are an integral part of the care team, collaborating with multiple healthcare professionals to ensure comprehensive and effective patient management. Your responsibilities include evaluating high-risk patients, coordinating care plans, and providing care management to improve patient outcomes.

8-hour shift (Monday‑Friday; We are a leading physician group serving South Carolina and Georgia, dedicated to delivering quality healthcare directly to patients in care facilities, homes, clinics, and virtual visits. Evaluate high-risk patients with complex medical needs and develop individualized care plans.

Coordinate care activities with other members of the interdisciplinary team to ensure continuity of care and optimal patient outcomes.

Provide patients with education and resources related to their conditions, treatment plans, self‑management strategies, and preventive care measures.

Offer support and guidance to patients and their families to enhance their understanding of healthcare options and promote active participation in their care.

Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.

Monitor quality measures and gaps in care, identifying opportunities for improvement and implementing corrective actions as needed.

Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required. Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.

Ability to evaluate high-risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes.

Basic computer knowledge.

Ability to foster a cooperative work environment.

Must be able to work flexible hours and travel between offices, facilities, etc.
