Physiatrist Physician (Physician )

Overview Pride Health is hiring a Physical Therapist to support our client's healthcare facility in West Monroe, LA.

Job Type: Contract

Contract Length: 13 weeks (possibility of extension)

Pay offered is based on experience, expertise, credentialing, and education.

Assess home environment for safety risks and fall hazards.

Complete required documentation including OASIS and physician orders.

Provide therapeutic exercises, gait and balance training, and functional mobility training.

Train caregivers on proper mobility and transfer assistance.

Coordinate care with physicians and interdisciplinary team members.

Participate in discharge planning and recommend ongoing care or services.

Requirements Louisiana PT license.

Home Health experience required.

Additional Requirements Ability to travel daily to patient homes within assigned territory (50 miles radius).

Comfortable working in home-based care environments.
