Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at Vineland. This full-time opportunity is ideal for a therapist who enjoys working with residents to restore movement, build strength, and improve overall function through personalized rehabilitation.

What You’ll Do

Complete physical therapy evaluations and develop individualized plans of care

Provide skilled treatment addressing strength, balance, gait, transfers, endurance, and functional mobility

Establish appropriate rehabilitation goals and track resident progress

Adapt treatment approaches based on each resident's needs and response to therapy

Educate residents and caregivers on exercises, mobility strategies, and safe movement techniques

Maintain accurate and timely clinical documentation

Coordinate care with PTAs, OT, SLP, nursing, and other members of the interdisciplinary team

Qualifications

Active New Jersey Physical Therapist license required

Graduate of an accredited Physical Therapy program

Strong clinical, communication, and interpersonal skills

Skilled nursing or post-acute rehabilitation experience preferred, but not required

New graduate Physical Therapists are welcome to apply
