Licensed Practical Nurse (LPN) - Long-Term Care

This is a full-time position with a schedule from 2:00 PM to 10:00 PM.

The Licensed Practical Nurse (LPN) will be responsible for providing high-quality care to residents in a long-term care facility. The LPN will work as part of a multidisciplinary team to implement patient care plans, assist with daily living activities, and ensure residents' comfort and safety.

Responsibilities

Provide nursing care in accordance with established policies and procedures.

Administer medications and monitor residents' health status.

Assist in coordinating care with interdisciplinary teams.

Document patient information and treatment plans accurately in the electronic medical record (EMR).

Participate in care team meetings and contribute to care planning.

Assist residents with daily living activities and mobility.

Required Experience / Certifications / Licensure

Valid and active LPN license in New York.

Minimum of 1 year of experience in a long-term care (LTC) facility.

Current Basic Life Support (BLS) certification.

Documentation of travel experience, if applicable.

Skills checklist matching specialty required.
