As a compassionate and dedicated Physical Therapist Assistant (PTA), you will play a vital role in helping patients achieve their rehabilitation goals and improve their quality of life. Working under the direction and supervision of a licensed Physical Therapist, you will provide coordinated care to patients of all age groups. Duties and Responsibilities:

Provide physical therapy treatments and interventions as outlined in the Plan of Care established by the supervising Physical Therapist.

Observe, monitor, and document patient responses to treatment and communicate findings to the Physical Therapist.

Assist patients in improving mobility, strength, balance, endurance, coordination, and functional independence.

Instruct patients and caregivers on prescribed therapeutic exercises, home exercise programs, and safety techniques.

Recognize changes in patient condition and promptly report concerns to the supervising Physical Therapist and care team.

Maintain accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.

Assist with evaluating and recommending durable medical equipment and adaptive devices as directed by the Physical Therapist.

Apply infection control principles to prevent contamination and ensure patient safety.

Participate in mandatory continuing education and in-service programs.

Required Skills/Abilities:

Strong therapy skills and knowledge of accepted physical therapy practices.

Excellent interpersonal and communication skills.

Ability to work effectively under the supervision of a licensed Physical Therapist.

Knowledge of durable medical equipment and assistive devices.

Strong organizational and documentation skills.

Ability to work a flexible schedule and travel locally.

Reliable transportation and valid auto liability insurance.

Required Education and Experience:

Graduate of an accredited Physical Therapist Assistant program.

Current Texas State License as a Physical Therapist Assistant.

Minimum one (1) year of experience in home health, acute care, rehabilitation, or a related healthcare setting preferred.

Current CPR certification preferred.

Physical Requirements:

Must be able to lift and/or move up to 50 pounds.

Frequent standing, walking, bending, reaching, and hand dexterity required.

Ability to assist patients with transfers, mobility activities, and therapeutic exercises.

Close vision, color vision, and depth perception abilities needed.

Work Location & Travel Requirements:

Home office location is MyCHN Home Health Services:4005 Technology Dr, Angleton, TX 77515, USA

This position requires regular travel to patient homes and other assigned locations throughout Brazoria, Galveston, and Harris Counties.
