Registered Nurse (RN) - Home Health (Visiting)

Job Type: Full-Time/Part-Time Per Diem; The RN travels to a patient's home to provide direct care services, maintain patient independence, and ensure continuity of care. Responsibilities include administering at-home IVs, changing dressings, cleaning wounds, performing in‑depth assessments, overseeing treatment plans, and updating physicians and the agency on patient status. Documentation and communication with the agency are conducted through Synergy Software.

Patients require care 7 days a week. A flexible schedule is requested: at least two-three days per week (or ten productivity units), one weekend per month, and two holidays per year (one winter and one summer).

Provide nurse visits to home‑health patients (home visits).

Perform initial, comprehensive assessments and document results; Obtain and document physician orders.

Initiate, re‑evaluate, and revise the plan of care in consultation with the Clinical Team Manager.

Coordinate care with the Clinical Team Manager, physicians, and other home‑care team members; Perform re‑certifications, resumption of care, transfers, and discharges as requested; Assess the need for additional team members (PT, OT, ST, MSW, HHA).

Perform Home Health Aide supervisory visits per regulatory requirements.

Demonstrate knowledge of regulatory requirements.

Current California RN license (required).

NSO Nursing License Insurance (required).

IV/Phlebotomy Certified (required).

Visiting RN for Home Health: 1 year experience (required).

Nursing: 1 year experience (required).

Registered nurse with current licensure to practice in California.

Graduate of National League for Nursing accredited school of nursing.

Minimum 2 years experience, at least one in public health or home‑care nursing (preferred).

able to supervise home‑health aides.

Work is primarily on the road traveling between patient homes.
