Pay: $20/hour

Position Overview

We are seeking a compassionate and dependable Palliative CNA to join our growing team. In this role, you will provide personal care and supportive services to patients in the home under the supervision of a Registered Nurse or therapist. The ideal candidate is caring, reliable, and passionate about helping patients maintain comfort and dignity in their own homes.

Key Responsibilities

• Assist patients with personal care, including bathing, grooming, hygiene, dressing, mobility, and transfers. 
• Follow written care instructions developed by the RN or therapist. 
• Assist with exercises, ambulation, range-of-motion activities, and therapy-related support. 
• Support medication self-administration as permitted by state regulations and agency policy. 
• Observe and report changes in patient condition to the RN or Supervisor. 
• Maintain accurate documentation of care and patient condition. 
• Provide household services essential to patient care and comfort. 
• Build positive, professional relationships with patients, families, and team members. 
• Participate in patient care conferences and interdisciplinary team communication. 

Qualifications

Required:

• High school diploma or GED. 
• Successful completion of a state-approved nurse aide training and competency evaluation program. 
• Active CNA certification in good standing. 
• Valid driver’s license and proof of current automobile insurance. 
• Reliable transportation. 
• Ability to participate in on-call rotation. 
• Not listed on the OIG Excluded Provider List. 

Preferred:

• 1 year of CNA, home care, hospice, or palliative care experience.
