Job Description

We are seeking an experienced Case Manager with acute care experience to join our team in Eureka, California. This role requires the ability to work every other weekend and offers a unique opportunity to explore nature with easy access to seven different state and national parks. The Case Manager will oversee patient care coordination, appeals, and denials, as well as discharge planning to ensure seamless transitions for patients.

Responsibilities

• Manage care coordination for patients in acute settings.
• Conduct needs assessments and facilitate admission processes.
• Implement discharge planning and compliance with HIPAA regulations.
• Coordinate with interdisciplinary teams to support patient needs across various age groups, including adolescents, adults, pediatrics, and geriatrics.
• Work with HMOs, IPAs, and advance directives processes.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience in case management or acute care.
• Knowledge of CMS regulations and prior authorizations.
• Current and valid state license in the relevant field is required.
• Epic charting system experience is essential.
• Experience with protective services and guidelines pertinent to case management.
