Hello Dr,

• Must have active FL Medical license and DEA prior to presentation
• Must have clean background and malpractice
• Clinicians must have the below reflected on their case logs in the last 2 years in order to be granted privs:
• 25 GYN cases with 5 major abdominal surgeries
• 50+ deliveries

• All candidates must have met the minimum shift availability listed on the job.
• CV must have all dates in MM/YY to MM/YY format
• Work History must be in Sequential order with most resent-on top
• Any Gaps over 30 days must be explained on the CV
• EPA (Please use the attached EPA. Its also in Trio under your Name & then click on Help)
• All Licenses, Boards and Certs with expiration dates must be listed on CV
• Anyone with malpractice must provide a NPDB
• Anyone with legal issues must provide supporting documents

• 1st possible date they would be available?
• How many shifts would they be able for per month?
• What month is their schedule completely open?
• Any shift preferences (Day, Night, Blocksetc)?
• Any dates in the next 3-6 months they cant work?
• Any Holidays in the next 3-6 months they would be open to work. ?
