About the job Locum - Physician - OB/GYN
Hello Dr,
- Must have active FL Medical license and DEA prior to presentation
- Must have clean background and malpractice
- Clinicians must have the below reflected on their case logs in the last 2 years in order to be granted privs:
- 25 GYN cases with 5 major abdominal surgeries
- 50+ deliveries
- All candidates must have met the minimum shift availability listed on the job.
- CV must have all dates in MM/YY to MM/YY format
- Work History must be in Sequential order with most resent-on top
- Any Gaps over 30 days must be explained on the CV
- EPA (Please use the attached EPA. Its also in Trio under your Name & then click on Help)
- All Licenses, Boards and Certs with expiration dates must be listed on CV
- Anyone with malpractice must provide a NPDB
- Anyone with legal issues must provide supporting documents
- 1st possible date they would be available?
- How many shifts would they be able for per month?
- What month is their schedule completely open?
- Any shift preferences (Day, Night, Blocksetc)?
- Any dates in the next 3-6 months they cant work?
- Any Holidays in the next 3-6 months they would be open to work. ?
