Medical Facilities is looking for Locum - Physician - OB/GYN in Margate, FL

JOB DESCRIPTION:

SUBMISSION REQUIREMENTS

Must have active FL Medical license and DEA prior to presentation
Must have clean background and malpractice
Clinicians must have the below reflected on their case logs in the last 2 years in order to be granted privs:
25 GYN cases with 5 major abdominal surgeries
50+ deliveries

AS A REMINDER:

All candidates must have met the minimum shift availability listed on the job.
CV must have all dates in MM/YY to MM/YY format
Work History must be in Sequential order with most resent-on top
Any Gaps over 30 days must be explained on the CV
EPA (Please use the attached EPA. Its also in Trio under your Name & then click on Help)
All Licenses, Boards and Certs with expiration dates must be listed on CV
Anyone with malpractice must provide a NPDB
Anyone with legal issues must provide supporting documents

Availability

1st possible date they would be available?
How many shifts would they be able for per month?
What month is their schedule completely open?
Any shift preferences (Day, Night, Blocksetc)?
Any dates in the next 3-6 months they cant work?
Any Holidays in the next 3-6 months they would be open to work. ?
