About the Role:

As a RNAC Registered Nurse Assessment Coordinator , you will be responsible for coordinating assessments and developing individualized plans of care for our patients. This will involve gathering medical information, conducting physical assessments, and working closely with our interdisciplinary team to ensure that each patient receives the best possible care. You will also be responsible for ensuring that all necessary documentation is completed accurately and in a timely manner.

RNAC Registered Nurse Assessment Coordinator Responsibilities:

• Coordinate patient assessments and develop individualized plans of care

• Gather and analyze medical information to inform patient care decisions

• Conduct physical assessments and provide patient education

• Collaborate with the interdisciplinary team to ensure seamless care delivery

• Maintain accurate and up-to-date patient records

• Provide exceptional patient care and support to our patients and their families

Requirements:

• Prior experience as a Registered Nurse

• A current licensure as a Registered Nurse in the State of Pennsylvania

• Strong assessment and analytical skills

• Excellent communication and interpersonal skills

• Ability to work effectively in a fast-paced environment
