Physical Therapist Outpatient

Job Summary:

The Therapy/Rehabilitation - Physical Therapist (PT) in an outpatient setting provides skilled therapeutic care to patients across hospital inpatient, swing bed, long-term care, and outpatient services. The role involves assessment, treatment, and rehabilitation planning to improve patient function and quality of life. The PT collaborates with interdisciplinary teams and adheres to facility policies to deliver high-quality care. A minimum of one year of experience and current BLS certification are required.

Responsibilities:

Provide physical therapy evaluation, treatment, and rehabilitation services to patients in outpatient and other care settings.
Develop and implement individualized treatment plans based on patient assessments.
Collaborate with interdisciplinary teams to coordinate patient care and optimize outcomes.
Document patient progress and maintain accurate clinical records.
Adhere to facility protocols and safety standards during patient care.
Participate in quality improvement initiatives and continuing education as required.

Required Skills & Certifications:

Current Physical Therapy license valid in the state of practice.
Basic Life Support (BLS) certification.
Minimum of one year experience in outpatient, acute care, or long-term care/skilled nursing facility settings.
Ability to perform physical therapy assessments and treatments independently.
Strong communication and teamwork skills.

Preferred Skills & Certifications:

Experience in multiple care settings including outpatient, acute care, and LTC/SNF.
Willingness to accommodate block scheduling.

Special Considerations:

No on-call responsibilities.
Clinician will not float outside of scope to meet facility needs.
Traveler candidates must reside at least 50 miles from the facility.
Previous permanent staff must have a minimum 3-month separation before traveling.
Time off requests are approved individually upon offer.
Housing provided with restrictions: no pets, typically single occupancy, cleaning required upon departure.

Scheduling:

Work schedule options include 5 days of 8-hour shifts or 4 days of 10-hour shifts during daytime hours.
Guaranteed 72 hours per two-week pay period, with possible call-off for one 8-hour shift per two weeks.
Open to accommodating block scheduling.
