Physical Therapist - Rehabilitation

Job Description

The Physical Therapist will effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care. You will develop appropriate plans of care following all regulatory and clinical practice standards. Utilizing standardized assessments and evidence-based practice, you will support clinical interventions and administer various procedures as part of the rehabilitation plan.

Responsibilities

• Assess patient progress and adjust treatments in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
• Communicate effectively with supervisors and health team members regarding patient progress, barriers, and treatment plans.
• Document all regulatory and clinically required information in the electronic documentation system.
• Participate in required meetings including patient care conferences and rehabilitation meetings.
• Supervise and support physical therapist assistants, rehabilitation aides, and students.
• Adhere to APTA and state practice acts, code of conduct, and ethics.

Required Experience / Certifications / Licensure

• 1 year of PT experience required; new graduates allowed if they have Level II experience.
• Experience in LTC or Rehabilitation required.
• Oklahoma State License is required.

Schedule

This position requires an 8-hour day shift, working 8 hours each day.
