Community Case Manager: Connect Families to Services
Proactive Community Services
Proactive Community Services is seeking a Case Manager to deliver community-based nonprofit services, coordinating across PCS programs and partners. You will conduct intake, assessments, and enrollment activities, and develop individualized service plans to help participants overcome barriers.
