Medical Social Worker Home Health Part-Time/PRN

Job Description

MSA Home Health , a proud member of the Medical Services of America family, is currently seeking an experienced Part-Time/PRN Home Health Social Worker (MSW) for our Home Health location in Saint Petersburg and Wesley Chapel (Pasco and Pinellas County), FL.

What You'll Do:

\n
• Provides social work services in accordance with the physician’s plan of treatment.
\n
• Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
\n
• Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
\n
• Serves as a liaison with other community resources.
\n

Job Requirements

\n
• Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
\n
• Currently licensed to practice as a social worker in the state of FL.
\n
• Minimum of one-year Home Health experience preferred.
\n
• Valid Driver's License.
\n
