Description

• Conduct evaluations in both inpatient and outpatient settings and create personalized treatment plans
• Implement physical therapy interventions that target mobility, strength, balance, and functional independence
• Collaborate closely with nurses, physicians, case managers, and other rehabilitation experts to ensure optimal patient outcomes
• Educate patients and families on therapeutic goals, discharge plans, and home exercise routines
• Maintain accurate documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to facilitate coordinated care

Education

• Doctorate in Physical Therapy (DPT) or a relevant degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., neurological, geriatric) are appreciated but not essential

Skills

• Previous experience in a similar role is strongly preferred
