Care Coordinator - Fertility (Virtual)

Care CoordinatorFrame is seeking a Care Coordinator with strong experience in supporting patients during preconception and fertility treatment.About the RoleWe are currently hiring a Care Coordinator to join our fully remote team. This is a unique, flexible position with a minimum of 20 hours per week and the potential to increase over time and/or convert to hire in a PT/FT position. In this role, you will:Empower individuals and couples to reach their family goals and help them understand the next steps on their journey.Effectively communicate to patients asynchronously, utilizing Frame tools and processes.Follow up on clinical recommendations and care plans that are personalized to each patient and their family goals.Collaborate with other members of the care team (Medical Director, Health Coaches, Nurse Practitioners) to ensure comprehensive support for patients at all stages of their journey.What We're Looking ForYou love a flexible schedule and have the ability to work at least 20 hours/week (ideally mornings Eastern/Central Time), and more when needed.Empathetic and non-judgmental. Fertility journeys can have their challenges and we're here to support our patients.Exceptionally detail oriented and organized, with excellent written and verbal communication skills.Experience supporting patients in telemedicine / virtual care setting, especially those in women's health or fertilityRequirementsExperience working in healthcare is required2+ years of experience with fertility, reproductive health, and/or women's health careB.S.
