Overview
Physical Therapist – Skilled Nursing Facility in Lancaster, New Hampshire. Travel - temporary assignment starting 08/31/2026 for 13 weeks. Weekly pay range $2,515-$2,554 per week with guaranteed hours of 35 per week. Shift time from 08:00 to 16:30. Work setting is a long-term care (LTC) nursing home environment.

Responsibilities
- Assess residents to determine PT needs and develop individualized treatment plans aimed at improving mobility, balance, strength, and functional independence
- Provide hands-on interventions and therapeutic exercises, including gait training and ADL (activities of daily living) training
- Monitor and document patient progress, adjust treatment plans as needed, and communicate outcomes with the interdisciplinary team
- Apply appropriate modalities and equipment, ensure safety, and educate residents and caregivers on home exercise programs
- Collaborate with nursing staff and therapists to support discharge planning and transitions of care
- Maintain accurate records in accordance with facility policies and regulatory requirements

Requirements
- Active Physical Therapist license in the state of New Hampshire
- Degree from an accredited PT program (DPT or equivalent)
- Minimum 1 year of clinical PT experience, preferably in skilled nursing or geriatric rehabilitation
- Current CPR/BLS certification
- Ability to work on-site in a long-term care setting and adhere to the stated shift schedule
- Eligibility to work in the United States (no visa sponsorship required)
