Job Overview Payment: $50.00 - $140.00 per visit. Job Type: W-2 Per Diem. Medical Specialty: Home Health. Work Location: On the road. We are a Joint Commission Accredited Healthcare Provider. We are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills. Role The RN travels to a patient's home to administer services and helps patients maintain their independence. Duties include administering at-home IVs, changing dressings, cleaning wounds, in-depth assessments, overseeing patient treatment plans, updating doctors and the agency about patients' health. Documentation and communication with the agency is done through Synergy Software. Availability Patients need care 7 days a week. Flexibility in scheduling is needed, but you must be able to work at least two to three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer). Responsibilities Providing nurse visits to home health patients (home visits) Performing an initial, comprehensive assessment and documenting everything & uploading to the agency portal Obtaining and documenting physician orders Initiating and regularly re-evaluating and revising the plan of care in consultation with the assigned Clinical Team Manager Coordinating care with the assigned Clinical Team Manager, physician, and other members of the home care team, informing them of significant changes in the patient's condition and needs Performing re-certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager; completing and submitting all related documentation Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA) Performing Home Health Aide supervisory visits per regulatory requirements Demonstrating knowledge of regulatory requirements Position Qualifications Visiting RN for Home Health: 1 year (Required) Nursing: 1 year (Required) Current CA RN License (Required) BLS from the American Heart Association and/or the American Red Cross (Required) NSO Nursing License Insurance (Required) IV/Phlebotomy Certified (Required) This position is currently accepting applications. #J-18808-Ljbffr
