What You'll Do:

• Effectively evaluate, plan, and implement treatment for patients with physical functioning disorders
• Utilize evidenced-based assessments and develop personalized care plans
• Administer therapeutic procedures including manual techniques, ambulation, exercises, modalities, and wound care
• Monitor and document patient progress, adjusting treatments as needed
• Educate patients, families, and caregivers on rehabilitation and discharge plans
• Record all documentation accurately within electronic health records
• Participate in team meetings, trainings, and in-services
• Support and supervise rehabilitation aides or students when necessary

What You Need:

• Valid Physical Therapist (PT) license
• Relevant experience in long-term care settings preferred
• Strong communication and patient care skills
• Ability to adhere to clinical standards, safety regulations, and ethical practices
• Proven ability to work effectively in a team environment
