Work Environment\n

This position offers a remote work setting, enabling you to provide care effectively and efficiently from the comfort of your own home while still connecting closely with clients and colleagues. The standard work hours for full-time employees are Monday to Friday, 8:00 AM to 4:30 PM, with some flexibility required to meet client needs. Part-time schedules can be negotiated based on your availability and the demands of our client base. On-call availability may be necessary to address urgent situations or provide support during off-hours.

Key Responsibilities\n

• Conduct comprehensive assessments to identify the psychosocial needs of seniors, taking into account emotional, mental, and social factors.

• Develop personalized care plans in collaboration with clients and their families, ensuring alignment with their individual goals and preferences.

• Provide counseling and emotional support to seniors, guiding them and their families through transitions such as retirement, health challenges, and loss.

• Advocate for clients, facilitating access to essential community resources and services that enhance their well-being.

• Engage with interdisciplinary teams, including healthcare providers and caregivers, to coordinate care and improve client outcomes.

• Conduct regular client visits to monitor their well-being and reassess their needs, ensuring their living situations remain safe and supportive.

• Address conflicts or communication barriers within families involved in the senior clients’ care, promoting a collaborative approach.

• Educate clients and families about community resources and available support systems that foster social engagement and enhance life quality.

• Maintain detailed and accurate documentation of client interactions, assessments, care plans, and progress notes in compliance with ethical standards and regulations.

• Stay informed about current trends in social work practices to ensure continuous professional development and compliance with relevant policies.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution is required.

• Current and valid state licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.

• A minimum of two years’ experience in a social work capacity, ideally within a community health or geriatric setting.

• In-depth knowledge of aging-related issues and available community resources relevant to seniors.

• Demonstrated skills in assessment, counseling, and problem-solving.

• Ability to connect with seniors and their families through empathy, active listening, and effective communication.

Compensation and Benefits\n

The compensation for this role is competitive, with an annual salary ranging from USD 80,000 to 100,000.
