JOB SUMMARY:
The Community Assistance Center (CAC) Case Worker provides direct support and assistance to individuals and families who are experiencing homelessness, at risk of homelessness, or facing financial hardship. The Case Worker assesses needs, processes applications for assistance, facilitates client-centered goal setting, helps clients navigate barriers, provides referrals to community resources, and advocates for clients, particularly in the areas of housing,
employment, financial stability, and community connection.

The Case Worker supports clients in building strong networks of support and developing the resources and skills needed to achieve greater stability and self-sufficiency. The position administers emergency financial and material assistance in accordance with funding and contractual requirements, CAC policies and procedures, and The Salvation Army standards.

The Case Worker provides services in a culturally responsive and respectful manner and demonstrates an understanding of the diverse communities served throughout Hawaiʻi, including the ability to incorporate relevant Hawaiian, Asian, and Pacific Islander cultural values and perspectives when supporting clients toward healthy, stable, and balanced lives.

ESSENTIAL QUALIFICATIONS:
• Bachelor’s degree in Human Services, Social Work, Education, or Psychology preferred, or equivalent work experience
• Minimum two years’ experience working with the low-income or homeless community.
• Knowledge of local and community-based social service organizations
• Understanding of the issues impacting Hawaii’s homeless community or populations in need of financial or material assistance
• Effective verbal and written communication skills
• Excellent customer service and problem-solving skills
•Proficient organizational skills with strong attention to detail
•Ability to work independently and as part of a team.
•Valid Hawaii Driver’s License

ESSENTIAL FUNCTIONS:
1. Conduct client intake, screening, assessment, and eligibility determination for financial assistance, material assistance, case management, and other available services.
2. Process financial and material assistance in accordance with funding requirements, program guidelines, and established policies and procedures.
3. Assist clients in obtaining and appropriately utilizing community resources, including housing, employment, recovery support programs, educational opportunities, and other relevant services.
4. Facilitate budget coaching and life skills sessions to help clients strengthen financial stability.
5. Provide crisis intervention, problem-solving, and appropriate referrals when clients experience urgent or complex needs.
6. Refer qualified client families who meet case management criteria to a Pathway of Hope (POH) Case Manager.
7. Participate in case consultations, team meetings, supervision, training, program evaluation, and continuous quality improvement activities.
8. Timely record clear, thorough, and up-to-date service data in the Homeless Management Information System (HMIS), WellSky, internal excel spreadsheets, and database software.
9. Assist with reporting, outcome measurement, and documentation required by funding sources and organizational standards.
10. Respect and adhere to client privacy guidelines (HIPAA, 42 CFR, The Salvation Army Social Services Code of Ethics, and relevant POH guidelines).
11. Maintain professional boundaries and protect client confidentiality in accordance with organizational policies, ethical standards, and applicable laws and regulations.
12. Serve as a liaison between the CAC, Salvation Army Corps and social service programs, government agencies, and community service organizations to strengthen service delivery and access to resources.
13. Participate in off-site community outreach events to promote and deliver CAC services.
14. Demonstrate cultural humility and sensitivity when serving individuals and families from diverse cultural, socioeconomic, and life experiences.
15. Attend department meetings and training courses as assigned.
16. Perform other duties as assigned.

DESCRIPTION OF WORK ENVIRONMENT:
Most work will be indoors at The Salvation Army Family Treatment Services or at various units and community partner locations. The Community Assistance Center Case Worker must be able to work independently and collaborate with a team. The type of clientele will vary in needs, disposition, personality, and circumstances. The Community Assistance Center Case Worker provides services per policies and procedures, without bias or discrimination. On a regular basis, community outreaches are scheduled which will require the use of sunscreen or sun protection. Attendance at trainings are required.
