Registered Nurse Case Manager

Proforma Health

Proforma Health is seeking an RN for Case Management to coordinate patient care and ensure accurate documentation in medical records. You will document care delivered, administer therapies, monitor for adverse reactions, and perform medication reconciliation. Work 8-hour shifts totaling 40 hours weekly, with a focus on patient safety and care quality.
