Job Description
Title: Physical Therapist - Rehabilitation

Schedule: Monday to Friday, 7:30 AM to 4:00 PM

Assignment Details:
- Contract Length: 26 weeks
- Guaranteed Hours: Not specified

Requirements:
- Active Hawaii State license
- Minimum of 2 years of experience in outpatient and acute settings working with adults
- BLS certification from the American Red Cross
- EMR proficiency with Net Health Therapy and Meditech

Roles & Responsibilities:
- Evaluate and re-evaluate assigned patients as either outpatient or inpatient.
- Plan and implement ongoing therapeutic physical therapy programs as prescribed by the ordering physician.
- Regularly update treatment plans and home exercise programs.
- Independently manage a full caseload and determine when re-evaluations are necessary based on insurance authorization.
- Work on Hawaii State and Federal Holidays as needed.

Compliance / Notes:
- Must arrive the Thursday before the start date for an employee health appointment on Friday.
- Do not accept the start date offered if unable to arrive on time.
