Description RN Care Managers, in the acute care setting, provide a variety of services to appropriately help progress patients through the continuum of care. Care Manager RN at Providence Portland Medical Center Positions Available: Full Time with 1 FTE with Day Shift Schedule Required Qualifications Nursing degree/diploma required upon hire. Bachelor’s degree in Nursing or higher within 3 years of hire. Oregon Registered Nurse License upon hire. 2 years Acute care hospital or community based experience. 1 year Experience with electronic medical records or documentation systems. 1 year Home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience.
