\n \nWe are hiring an Admission Registered Nurse (RN) for a Home Health position providing skilled nursing care to patients in their homes in the Monongahela , PA area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!

\n \nWhat You’ll Do:

Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
\n
- \n
- \n
Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders
\n - \n
Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported from day one
\n - \n
Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation
\n - \n
Initiate evaluations to other disciplines when indicated
\n - \n
Work independently, but never alone
\n - \n
Travel to patient homes within assigned territory

Admission RN Qualifications
\n- \n
- \n
Active Registered Nurse, RN license in the state you work
\n - \n
One year of RN experience and the clinical expertise to deliver quality patient care
\n - \n
Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
\n
