Addus Family Company seeks a Hospice Weekend Admission RN to support home-based hospice patients across our service area. In this role, you’ll complete weekend admissions, conduct thorough nursing assessments, and initiate individualized plans of care. You will educate patients and families on hospice services, manage symptoms, and coordinate with our interdisciplinary team. Working within a mission-driven, patient-focused culture, you’ll provide compassionate end-of-life care, ensure accurate documentation, and uphold quality, safety, and regulatory standards while making a meaningful difference in patients’ lives.
