Advocate Health in Illinois is seeking an experienced Registered Nurse (RN) to join the Hospice Admissions team in the Oak Brook/Lombard area. You will lead the interdisciplinary care team for your patient caseload, assess needs, obtain informed consent, and coordinate services with physicians and the IDT to ensure seamless hospice care.
