JourneyCare Hospice in Vernon Hills, IL seeks a compassionate Hospice RN Case Manager to deliver patient-centered end‑of‑life care with a manageable caseload and strong interdisciplinary support. This full‑time role offers Mon–Fri 8:30 AM–5:00 PM, no on‑call, and a weekend rotation every 12 weeks.
