Hospice RN Case Manager — Care Coordination Leader

Purposelink Recruitment is seeking a compassionate Registered Nurse Case Manager to join our hospice team in Northbrook, IL. You will provide essential care coordination, leadership, and patient advocacy for hospice patients, collaborating with interdisciplinary teams to ensure exceptional end-of-life care. The schedule is Mon–Fri, 8:00 AM–5:00 PM with on-call coverage and stipends.
