LTSS Care Coordinator - Hybrid In-Home Case Manager

Meridian of Illinois

Centene is seeking a Hybrid LTSS Case Manager to support Aging and Physical Disabilities Waiver members in Illinois. You will conduct in-home assessments, develop and coordinate care plans, and connect members with providers, resources and DME services. Requires Bachelor's degree and 1 year of related experience; IL RN or LPN with relevant experience also considered.
