Luxe Home Health is a leading provider of high-quality home healthcare services. Our dedicated teams work together to deliver compassionate, personalized care that helps patients heal at home. We're growing and seeking a Care Coordinator to join our in-office team in Des Plaines.

Position Summary:

This full-time, in-office role plays a key part in ensuring seamless coordination of patient care. You’ll support clinical staff by processing new referrals, verifying insurance, and managing critical documentation. The role requires strong organizational skills, attention to detail, and excellent communication.

Please note: This is a 100% in-office role (no hybrid or remote options).

Key Responsibilities:

• Process Referrals: Handle incoming patient referrals and verify insurance eligibility.
• Manage Documentation: Collect and upload all necessary paperwork, including MD notes, lab results, and orders.
• Coordinate with Providers: Communicate with hospitals, physicians, and rehab centers to gather patient info.
• Support Clinical Teams: Ensure field staff have accurate, timely updates regarding evaluations, services, and DME.
• Communication Management: Handle a high volume of emails, calls, and faxes efficiently.
• Weekend On-Call: Provide support for urgent care coordination needs during assigned weekends.
• Deliver Excellent Service: Act as a professional, courteous liaison between patients, providers, and team members.

What We’re Looking For:

• Medical or clinical background (e.g., LPN, RN, MA, PTA, coder) strongly preferred
• Home health experience required
• Strong multitasking and time management skills
• Excellent verbal and written communication
• Comfortable with EMR systems and tech-savvy
• Team player who can also work independently

Schedule & Benefits:

Schedule: Monday–Friday, 8-hour day shifts
On-Call: Weekend availability required
