Home Care CoordinatorMySolutions Home Care, part of Lutheran Life Communities, is hiring a Home Care Coordinator in Arlington Heights. $20.00–$21.00/hour, credit for experience. Full-time, Monday–Friday, 8:00 a.m.–4:00 p.m., with occasional evenings and weekends.As a Home Care Coordinator, you'll manage intake and care coordination for clients receiving MySolutions Home Care services, helping residents and families navigate care transitions smoothly, from initial intake through ongoing home visits.What You'll DoManage intake calls and follow up on incoming leads for MySolutions Home Care servicesVisit new residents in MyRehab shortly after admission and again before discharge to support smooth care transitionsAssess client needs and develop individualized service plansOversee delivery of home care and handyman servicesConduct regular home visits, 90-day check-ins, annual care plan reviews, and ongoing assessmentsBuild strong relationships with clients, families, and caregivers, and connect them with community resourcesAssist with caregiver scheduling, onboarding, and trainingWhat You'll NeedRequired:High school diploma or equivalentValid driver's license, for regular home visits across the service areaAble to read, write, and comprehend English instructions and documentationBasic math and computer proficiencyPreferred:College degreeBackground in healthcare, social work, sales, or senior livingSupervisory experiencePay and Benefits$20.00–$21.00/hour, credit given for experienceMedical, dental, and vision insuranceFlexible Spending Accounts (FSA)403(b) retirement plan with employer matchLife and AD&D insuranceShort- and long-term disability coverageAccident, critical illness, and hospital indemnity insuranceEmployee Assistance Program (EAP)Wellness programsSchedule Full-time, Monday–Friday, 8:00 a.m.–4:00 p.m., occasional evenings and weekends as needed.You'll be helping residents and families navigate care transitions with MySolutions Home Care in Arlington Heights.
