Role Description This is a full-time, on-site role for a Social Worker located in Westborough, MA. The Social Worker will be responsible for providing emotional support and counseling to patients and families, developing care plans, and facilitating access to community resources. Additional day-to-day tasks include communicating with healthcare teams, advocating for patient needs, and assisting with discharge planning to ensure smooth transitions of care. Qualifications Strong counseling, communication, and interpersonal skills to provide effective emotional support and build trust with patients and families Experience in care planning, discharge planning, and identifying appropriate community resources Ability to collaborate with interdisciplinary healthcare teams and advocate for patient needs Understanding of healthcare regulatory policies and social service practices Excellent organizational and documentation skills Bachelor’s or Master’s degree in Social Work (MSW preferred) Licensed Clinical Social Worker (LCSW) or ability to obtain licensure in Massachusetts Prior experience in a rehabilitation or healthcare setting is a plus #J-18808-Ljbffr
