Outpatient Physical Therapist

This is a 13-week contract position.

Shift: 5x8.0

The Outpatient Physical Therapist will evaluate assigned patients and develop, implement, and modify treatment plans according to the patient's needs, goals, and physician directions. Responsibilities include delegating therapy services, designing functional treatment programs, and serving as an educator to patients, families, students, and colleagues. Adherence to safety standards and compliance with professional codes of conduct is essential.

• Complete comprehensive evaluations of assigned patients.
• Develop treatment plans and goals focusing on patient progression towards discharge.
• Document medical records accurately and timely, ensuring compliance with state and federal guidelines.
• Participate in student programs, providing supervision and evaluations.
• Create individualized professional development plans and pursue ongoing education.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure.
• Engage in performance improvement initiatives to enhance service quality.

• Degree in Physical Therapy from an accredited school.
• 1+ years of outpatient clinical experience.
• Current CPR certification.
• Active license for the state where work will be performed.
