Our mission is to provide and support quality care for our veterans, servicemembers and their families, across the United States.Job Summary: The RN Case Manager provides comprehensive case management services through patient assessment, care planning, coordination, implementation, monitoring, and evaluation to promote continuity of care and optimal health outcomes.Required Qualifications:Education: Associate Degree in NursingCertifications: Basic Life Support (BLS)Certified Case Manager (CCM) orCertified Disability Management Specialist (CDMS) orCertified Rehabilitation Registered Nurse (CRRN) orCertified Occupational Health Nurse (COHN) orCertified Occupational Health Nurse-Specialist (COHN-S) orAdvanced Certification in Continuity of Care (ACCC) orCertified Rehabilitation Counselor (CRC) orNurse Case Manager (RN-NCM) orCare Manager Certified (CMC)License: Current, full, active, and unrestricted license to practice as a Registered Nurse.Experience: Associate Degree in Nursing holders: Minimum of four (4) years of experience in discharge planning or case management.Bachelor of Science in Nursing graduates: Minimum of two (2) years of full-time experience in discharge planning or case management. Full-time employment in a nursing field within the last 36 months is required.U.S. Citizenship is requiredKey Responsibilities:Provide nursing expertise on the case management process, including assessment, planning, implementation, coordination, monitoring, and evaluation.Develop and implement local strategies utilizing inpatient, outpatient, onsite, and telephonic case management.Develop treatment plans that include preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care and optimal wellness.Establish mechanisms for proper implementation of patient treatment plans and follow-up care post-discharge in ambulatory and community healthcare setting.Work Environment & Schedule: Onsite, Monday - Friday, 7:00am - 4:30pmPhysical requirements: Ability to sit or stand for extended periods while reviewing medical records and working on a computer. Frequent use of standard office equipment and use of clear verbal and written communication.
