If you’re a nurse who values teamwork, resident-centered care, and a supportive work culture, we’d love to meet you.What You’ll Do:Provide clinical oversight and clear direction to caregiving staffAssess residents, document changes, and communicate with physicians and familiesPerform diabetic care, medication tasks, and follow physician ordersCoordinate medical appointments and support proactive health managementPartner with Home Health/Hospice teams to ensure comfort and continuity of careTrain, coach, and support caregiving staffUphold infection control and regulatory standards (Title 22)Minimum QualificationsCurrent California LVN licenseCurrent CPR and First Aid certification2 years nursing experience (assisted living, memory care, or SNF preferred)Strong communication, leadership, and teamwork skillsKnowledge of gerontology, dementia care, and resident rightsAbility to remain calm and effective in emergenciesWhy Join Silvergate?We offer a positive, collaborative culture and a mission‑driven commitment to exceptional resident care.
