Whether you're an experienced home health clinician or a therapist looking to transition from outpatient or another clinical setting, you'll be supported by an experienced interdisciplinary team committed to delivering exceptional patient care. What You'll Do As a Home Health Physical Therapist, you'll provide skilled physical therapy services under a physician-directed plan of care, helping patients safely recover from illness, injury, surgery, or hospitalization while maximizing their independence at home. Your responsibilities will include: Performing comprehensive evaluations, reassessments, start-of-care visits, recertifications, and discharges Developing individualized treatment plans focused on each patient's functional goals Providing evidence-based interventions to improve strength, balance, mobility, endurance, and safety Educating patients and caregivers on home exercise programs, fall prevention, disease management, and safe mobility Collaborating closely with nurses, physicians, occupational therapists, speech therapists, and other members of the interdisciplinary care team Completing accurate, timely clinical documentation, including OASIS assessments Monitoring patient progress and modifying treatment plans to achieve the best possible outcomes What We're Looking For Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate) Current Connecticut Physical Therapist license Minimum of two years of clinical experience preferred Home health experience and OASIS documentation experience preferred, but not required Excellent clinical assessment, communication, and time management skills Ability to work independently while collaborating as part of a multidisciplinary team Reliable transportation and a valid driver's license Why Physical Therapists Choose RVNAhealth At RVNAhealth, you'll have the opportunity to practice physical therapy the way it was meant to be-focused on the whole person, not productivity metrics. Compensation: The full time pay range for this position is $90,000 to $110,000 per year and represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
