Remote Telephonic Nurse Care Manager
Remote Services Inc.
Humana seeks a Care Manager, Telephonic Nurse, for a remote role coordinating care for Medicaid/Medicare enrollees. You will assess needs, create care plans, and guide members toward resources to improve wellbeing.
Work with an interdisciplinary team, perform telephonic and in-person assessments as needed, and travel to Humana Oklahoma offices for meetings and training. Requires Oklahoma residence, active RN/LPC/LCSW license, and 2+ years in care management.
