Job Details – Location: Albuquerque, NM (Alburquerque) – Office – Position Type: Part Time – Education Level: 2 Year Degree – Travel: Road Warrior – Shift: Any Summary This position assists in the implementation of the rehabilitative plan of care under the direction and supervision of a qualified physical therapist. Essential Duties and Responsibilities Upon assignment by the PT, contact the patient within 24 hours and begin therapy as prescribed. Administer therapeutic exercises and techniques delegated by the Physical Therapist. Assist in instruction of home exercise programs to patient and family. Assist patients in use of assistive devices. Consult with Physical Therapist regarding outcome of home visits and patient progress. Participate in department staff meetings, case conferences, and in‑service programs. Assume other duties or attend other meetings as deemed appropriate by supervising Physical Therapist or Regional Director. Practice in a manner consistent with the professional and Harmony code of ethics. Follow Harmony’s adverse event/incident reporting procedure and Emergency Preparedness Plan. Establish and maintain effective communication and professional relationship with client, families, and coworkers. Provide documentation of patient care per established criteria, including regular case conference notes and submission of caseload rosters. Mantain confidentiality in relation to all patients, healthcare staff and documentation; seek direction if asked to share any patient information with outside parties. Utilize universal precautions and follow OSHA guidelines. Maintain and enhance skills through continuing education. Appear professional and comply with the Company dress code. Fulfill assignments reliably and punctually. Listen and respond to clients’ needs in a sensitive and timely manner or refer to Case Manager or supervising Physical Therapist. Qualifications and Experience Graduate of a Physical Therapy Assistant program approved by the American Physical Therapy Association. Registration or certification in the practicing state. Preferably one year of experience in a health care setting. Ability to read and follow written instructions and document care. Self‑directed with the ability to work with little direct supervision. Demonstrates tact, patience, and good personal hygiene. Complete and pass skills evaluation test annually. Effective written and oral communication skills and good interpersonal skills. Current CPR card, driver’s license, auto insurance, safe driving record, and reliable transportation. Successful completion of a criminal background check. Physical Demands and Work Environment While performing duties, the employee must: lift, stand, bend, transfer, stoop, stretch, walk, push, pull, talk, hear, and assist with activities of daily living. May need to lift or move up to 60 pounds or support patients weighing more. Must meet vision abilities: close, distance, color, peripheral, depth perception, and focus adjustment. Physical demands also include partial or complete assistance with ADLs without another provider’s help. Work environment includes exposure to weather conditions prevalent at the time.
