Position Full‑time Physical Therapist Case Manager Anticipated Territory: Folsom Area Responsibilities Perform an evaluation in the patient’s home and develop a care plan while working with Physical Therapy Assistants (PTAs) to follow the plan. Assist patients with mobility and movement, helping them regain independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure high‑quality care. Evaluate and report patients’ progress to medical doctors. Competitive compensation with incentive bonus program. Qualifications Current unrestricted CA Physical Therapist license. Current BLS/CPR certification. Valid CA driver’s license. Knowledge of local community resources. Understanding of injury and illness impact and cultural influences on health care. Knowledge of state and federal home‑care regulatory guidelines preferred. Experience in home health care highly preferred. Experience with high acuity patients a plus. Experience working with an interdisciplinary team highly desired. Case management fundamentals highly desired. Self‑directed, empathetic, critical thinker, with outstanding interpersonal and leadership skills. Organized. Salary: $120,000 – $135,000 a year, plus up to $20,000 annual bonus depending on experience and performance.
