Physical Therapist Case Manager, Level 2: Home Health

We offer full-time roles such as Physical Therapist Case Manager, Level 2, with anticipated territory in the Hayward, CA area. Responsibilities Perform an evaluation in the home, develop a care plan, and work with PTAs to follow the care plan. Help patients with mobility and independence, and create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure a quality level of care. Evaluate and report patients’ progress to the MD. Supervise 2 Physical Therapy Assistants (PTAs) to ensure they meet performance standards. Qualifications Current unrestricted CA Physical Therapist License 1+ year experience supervising PTAs in a home health environment is a plus Current BLS/CPR Certification Valid CA Driver's License Knowledge of local community resources Knowledge of development, behavior, and how culture influences health care Knowledge of state and federal homecare regulatory guidelines (preferred) Experience in home health care and with high-acuity patients (preferred) Experience working with an interdisciplinary team; case management fundamentals (highly desired) Self-directed, empathetic, critical thinker with outstanding interpersonal and leadership skills Organized with a strong work ethic Indeed platform tag: 21hhsmp Benefits and Compensation Salary: $125,000 – $160,000 per year Annual bonus opportunity (e.g., $20K) and starting pay commensurate with experience Generous benefits: medical, dental, life, EAP, FSAs, and more 401(k) with matching PTO and competitive incentive programs Company issued device (iPad or laptop) and phone number; mileage reimbursement for visits Flexible schedules, territory-based assignments to minimize driving, and autonomy (no clocking in/out) Additional notes: Prior to scheduling an in-person interview, candidates may complete a computer literacy and competency assessment to ensure success with electronic charting and company systems.
