• Deliver relationship-focused primary care to adults age 65+
• Practice in a value-based care model designed around quality—not RVUs
• Build meaningful patient relationships with longer appointment times
• Work alongside an integrated multidisciplinary care team
• Focus on preventive medicine and chronic disease management COMPENSATION
• Base Salary: $235,000 - $270,000
• Total First-Year Compensation Up To $300,000+
• Annual Performance Bonus Up To 20%
• Commencement Bonus Up To $75,000
• Relocation Assistance Available (50+ Miles) SCHEDULE
• Full-Time
• Outpatient Primary Care ROLE
• Care for adults 65 years and older
• Average 12-15 patients per day
• 20-40 minute appointment times
• Collaborate with NPs, PAs, Medical Assistants, Social Workers, Pharmacy, Lab, and Referral Coordinators
• Deliver preventive care while managing acute and chronic conditions
• Participate in an integrated value-based care model ENVIRONMENT

• Board Certified In Internal Medicine Or Family Medicine REQUIRED
• Active Medical License REQUIRED

• No Residents Or New Graduates

• HEDIS Experience Preferred
• Value-Based Care Experience Preferred (Training Available)
