Advanced Practice ProviderThe Cosumnes Community Services District is seeking part-time year-round Advanced Practice Providers (Physician Assistants or Nurse Practitioners) to join our team. Candidates in this position will work 20 hours or less per week. Actual hours assigned may vary depending upon the needs of the division.Under general supervision, is responsible for providing patient-centered care in non-traditional healthcare settings, bridging gaps between hospital, primary care, and community-based services. Focuses on preventative care, chronic disease management, post-hospitalization follow-ups, and acute interventions, ensuring patients receive timely, cost-effective care in their homes or community environments.The salary range for the Advanced Practice Provider (Physician Assistant or Nurse Practitioner) classification is paid at $85.00 per hour.Duties and ResponsibilitiesThe following duties are intended only as illustrations of the various types of work that may be performed. Employees may perform other related duties at an equivalent level as required.Perform comprehensive assessments, including physical exams, patient histories, and diagnostic evaluations in community, healthcare, or home settings.Coordinate with the patient's Primary Care Physician (PCP) or Health Care Provider (HCP) to develop, implement, and/or adjust individualized patient care plans.Provide treatments, prescribe medications, and order diagnostics as needed, in accordance with state regulations.Conduct follow-ups to monitor patient progress and address emerging health issues.Educate patients and caregivers on disease management strategies.Coordinate care with specialists and primary care providers to optimize health outcomes.Monitor and adjust treatments for conditions such as diabetes, hypertension, heart failure, and COPD.Address urgent care needs that do not require emergency department visits.Provide triage and initiate immediate interventions in collaboration with emergency medical services (EMS) when needed.Promote health literacy by educating patients and families about wellness, prevention, and available resources.Serve as a liaison between patients and healthcare providers, ensuring seamless communication.Collaborate with social services, public health agencies, and other community resources to address social determinants of health.Participate in outreach initiatives to identify and assist underserved or high-risk populations.Document patient encounters, care plans, and outcomes in electronic medical records (EMRs).Analyze and report on patient care metrics to support program improvement and stakeholder communication.Work closely with a multidisciplinary team, including physicians, nurses, EMS providers, law enforcement, social workers, and behavioral health specialists.Participate in case conferences to discuss complex cases and strategize patient care.Provides in-home safety assessments to prevent falls and injuries in high-risk populations (e.g. elderly, disabled individuals).Performs other related essential duties as required.QualificationsAny equivalent combination of training and experience which provides the required skills, knowledge, and abilities may be considering qualifying at the sole discretion of the District. A typical way to obtain the knowledge and skills would be:A license to practice as a Physicians Assistant or Nurse Practitioner in the state of California and maintain it throughout employment.Current certification through the National Commission on Certification of Physician Assistants (NCCPA) orCurrent certification through the American Academy of Nurse Practitioners Certification Board (AANPCB).2 years of recent professional experience as a Physicians Assistant or Nurse Practitioner.Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification.Experience in primary care, emergency medicine, or community health preferred.Experience in telemedicine platforms and mobile healthcare delivery preferred.Must possess a valid California class "C" driver's license, with a clean driving record and maintain it throughout employment.Knowledge of:Principles, practices, and procedures of emergency medicine and medical care in emergency and non-emergency environments.Principles and practices involved in providing emergency medical services for ill/injured patients at the scene or en-route as part of a care teamPrinciples and practices associated with monitoring prehospital provider competencies and protocol compliance.Local EMS Agency (LEMSA) protocols, function of private and public ambulance providers and hospital emergency department operations.Variety of computer programs and applications necessary to perform the job.Methodology of data collection, evaluation, and interpretation.Ability to:Effectively and tactfully communicate both orally and in writing with CSD personnel and the public, representatives from other government agencies, independent contractors, vendors, etc.Communicate effectively with patients, families, caregivers, and interdisciplinary teams to ensure coordinated, patient-centered care.Establish and maintain effective working relationships with those contacted in the course of work.Objectively monitor performance of paramedics.Function independently in non-traditional healthcare environments with minimal direct supervision, while maintaining adherence to clinical protocols and regulatory standards.Comprehend and correctly utilize mobile technology, electronic health records (EHR's), and Telehealth platforms for documentation, consultation, and follow-ups.Prioritize multiple tasks.Demonstrate flexibility, cultural sensitivity, and professionalism while working in diverse community setting and under variable conditions.Prepare clear and concise reports, correspondence and memos using prescribed format and conforming to all rules of punctuation, grammar, diction and style.Mobility: Frequent standing, walking, climbing stairs, bending, kneeling, and squatting for extended periods; capable of lifting, pushing, or pulling equipment and/or patients up to fifty (50) pounds with or without assistance; ability to move quickly and safely in a variety of environments, including homes, multi-story residences, and uneven outdoor terrain;Vision: Constant use of overall vision; Frequent near and far visual acuity for physical assessment, reading charts or screens, and operating diagnostic equipment; must have color vision sufficient to recognize skin color changes, medication labels, and test indicators;Dexterity: Frequent use of fine motor skills and hand-eye coordination to perform clinical procedures (e.g., venipuncture, wound care, medication administration); Frequent ability to write, type, and use handheld and mobile medical devices;Hearing/Talking: Frequent hearing and talking in person and on the phone; Hearing and understanding normal speech, auscultatory sounds (e.g., heart, lungs, bowel), and alarms or other signals; requires the ability to speak clearly and effectively with patients, families, and team members, both in person and via phone or telehealth systems;Emotional/Psychological: Frequent public and/ or coworker contact; Requires resilience and emotional stability to manage high-stress situations, including critically ill patients, emotionally distressed individuals, or traumatic environments; occasionally works alone;Driving: Ability to use fine and gross motor coordination for driving, must be able to safely enter/exit a vehicle multiple times per shift and carry equipment to and from patient locations.Typical Working ConditionsWork is typically performed in a indoor and outdoor setting such as homes, businesses, or public spaces, and will vary from clean and organized to chaotic and unsanitary; extremes of weather such as heat, cold, rain, and wind; potential exposure to health risks such as bodily fluids or infectious diseases; emotionally and psychologically charged environments; frequent interactions with other EMS providers on-scene of emergent and non-emergent incidents which can be dynamic, unpredictable, and high-stress. Work requires travel to and from off-site locations throughout the District.Supplemental InformationThe process includes application review, supplemental questions, supplemental screening, and virtual Zoom interviews.
