Community Outreach Specialist
The MetroHealth System
The MetroHealth System in Cleveland, OH is seeking a Community Health Worker to support patients in the clinic and surrounding community by promoting health and reducing disparities.
You will screen for social determinants, connect patients to eligible resources, and build trusting relationships with patients, caregivers, and community partners. The role supports equitable access to care and aligns with MetroHealth’s mission.
