About This Role The Home Health Patient Care Manager is responsible for the overall supervision and coordination of clinical services, ensuring high-quality care for home health patients in accordance with physician prescribed plans and applicable regulations. Highlights Pay: $85,196 - $127,794 annually Location: Metairie, LA Facility: Ochsner Home Health of New Orleans Brand/Division: Optum Schedule: Full Time What You'll Do Provide clinical services within the scope of practice as defined by state laws. Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits. Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals. Ensure patient needs are continually assessed and care rendered is individualized. Review assessments and plans of care daily and consult clinicians with recommendations. Function in any home situation regardless of client demographics.
