Your Responsibilities

• Carry out thorough patient evaluations, order and interpret diagnostic tests, and create tailored care plans
• Diagnose and manage acute, chronic, or episodic health conditions, both independently and in collaboration with physicians as needed
• Educate patients and family members on health promotion, prevention strategies, and effective disease management
• Prescribe medications and therapies while adhering to the regulatory scope of practice
• Engage in chronic care management and routine patient follow-ups
• Work collaboratively as part of a multidisciplinary team to provide comprehensive and continuous care
• Maintain detailed and precise documentation through electronic medical records (EMR)

Necessary Qualifications

• Master of Science in Nursing (MSN) or a Doctor of Nursing Practice (DNP)
• Graduate from an accredited Nurse Practitioner program

Mandatory Certifications

• Valid Nurse Practitioner license in California
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or equivalent specialty credential
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification, or the ability to obtain these within 90 days
