JOB SUMMARY: The Home Service Worker provides non-medical personal care and supportive services to clients in their homes. This role assists clients with activities of daily living (ADLs), promotes independence, and ensures a safe, comfortable home environment while maintaining dignity and respect.

Personal Care Services

• Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, and toileting.


• Help with mobility, transfers, and ambulation as directed in the care plan.


• Assist with meal preparation, feeding, and maintaining proper nutrition and hydration.


• Provide medication reminders as permitted by agency policy.


• Monitor client condition and report changes to the supervisor promptly.


Companion and Homemaking Services

• Provide companionship and emotional support to clients.


• Assist with light housekeeping, laundry, and maintaining a clean living environment.


• Assist with shopping, errands, and transportation assigned.


• Support clients in maintaining independence and daily routines.


Safety and Client Support

• Maintain a safe environment and follow established safety procedures.


• Observe and report any safety hazards, changes in client condition, or concerns to the supervisor.


• Follow infection control practices and proper hygiene standards.


• Respect client rights, privacy, and confidentiality always.


Documentation and Compliance

• Document services provided according to agency policy and procedures.


• Follow the individualized care plan and agency guidelines.


• Maintain compliance with applicable state regulations, including standards established by the Illinois Department of Public Health (IDPH), when applicable.


• Attend required training and in-service education sessions.

Requirements

JOB REQUIREMENTS:


• High school diploma or equivalent required.


• Previous experience in home care, caregiving, or related field preferred.


• Experience assisting with personal care and activities of daily living preferred.


• Licensure/Certification:

• Certification as a Personal Care Assistant (PCA) or Homemaker preferred, if required by state or agency policy.


• Valid driver's license and reliable transportation preferred (if transportation duties are required).

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:


• Knowledge of basic personal care and homemaking services.


• Understanding safety practices and infection control procedures.


• Knowledge of client rights and confidentiality standards.


• Ability to assist clients with activities of daily living safely and respectfully.


• Strong communication and interpersonal skills.


• Ability to follow care plans and instructions accurately.


• Ability to recognize and report changes in client condition.


• Ability to maintain professionalism, compassion, and respect for clients.


• Ability to work independently in a client home setting.

• Ability to work collaboratively with clinical and administrative teams.


• Ability to read, write and speak English sufficient to perform job duties and to interact and communicate with residents, family members and employees.

ENVIRONMENTAL AND PHYSICAL ACTIVITY REQUIREMENTS : The physical activities of this position constantly require effective talking, hearing, sitting, walking and standing functions. The position also often involves sedentary work where sitting is required most of the time, with occasional standing or walking. The position also often requires exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Must be able to perform activities with close visual acuity, such as reading a computer screen and reading print material. The functions of this position are conducted in a temperature-controlled environment.

OSHA Exposure Category: Category III - Position includes tasks that involve no exposure to blood, body fluids, tissues; would not be required to perform Category I tasks.

INTELLECTUAL/EMOTIONAL REQUIREMENTS: The occupant of this position must be able to perform the responsibilities of the position well under pressure. The position requires adaptability to performing a variety of duties, often changing from one task to another without loss of efficiency or composure. This position requires the ability to deal with and find resolutions to problems and maintain a productive working relationship with employees and may be interrupted frequently to meet the needs and requests of others. It also requires adaptability to fluctuate between priorities daily while being responsive to needs as they arise.
