Apply now and a recruiter will reach out with more details. Minimum Requirements Master's (MPT) or Doctor of Physical Therapy (DPT) degree from an accredited program. Active Colorado Physical Therapy license required to start the assignment. Home health experience with OASIS documentation required. Experience performing comprehensive patient evaluations, developing measurable plans of care, and documenting skilled necessity for reimbursement. Proven ability to provide skilled PT interventions in home settings, including therapeutic exercise, gait and balance training, transfer and stair training, and assistive device training. Experience educating patients and caregivers on home exercise programs, fall prevention, energy conservation, and caregiver transfer techniques. Experience coordinating with interdisciplinary teams and communicating with physicians regarding plan of care and discharge planning. Daily travel to patient homes within an approximately 50-mile service radius. Ability to travel to multiple locations within a shift required. Preferred: Experience with HCHB electronic documentation system. Certifications: BLS from AHA or ARC may be required. Assignment Details Facility Type: Home Health. Shift Name: Days. Shift Details: Days 0800-1600. Work Days Per Week: Monday through Friday, five days per week. No call. Travel Requirement: Daily travel to patient homes within the agency service area; may visit multiple patient locations per shift. Dress Code: Scrubs in blue or black.
