Active Colorado PT license required to start the assignment. BLS certification from AHA or ARC may be required. Home Health OASIS Clinician experience required. Ability to provide services across a coverage area up to a 50 mile radius of Fort Morgan. Experience with home-based skilled PT interventions including therapeutic exercise, gait and balance training, transfer training, functional mobility, pain management, and fall prevention. Experience completing required home health documentation (OASIS components, visit notes, reassessments, discharge summaries) and communicating with interdisciplinary care teams. Preferred: HCHB (Homecare Homebase) electronic documentation experience. Assignment Details Facility type: Home Health. Shift name: Days. Shift schedule: Monday–Friday, 0800–1600 (days). Weekly hours: 40. Productivity expectation: 30 points. Coverage area: Up to 50 mile radius of Fort Morgan; mileage reimbursement provided between visits from first patient to last patient. Certification required: MPT or DPT. Minimum education requirement: Master's degree. On call/weekend requirement: None.
