DDS Day Hab Support Staff - Relief Per Diem - Community Connections

Overview As a Per Diem Day Hab Support Staff, you will provide support to individuals served in a direct care role by ensuring a supportive and structured environment. You will also provide guidance, advocacy, and life skills education for individuals served within the Day Program. Responsibilities Assist in developing individualized, person-centered treatment plans and play a lead role in the daily implementation plans. Assist individuals to access and/or provide transportation. Implement activities that create opportunities for the development of valued roles and personal relationships in the community. Review and complete all required documentation. Qualifications High School Diploma, GED or equivalent, required. Valid Driver's License and acceptable driving record, required. Reliability, willingness to learn, and being open to new opportunities.
